*Responsibilities*
- Conduct thorough patient assessments, including medical history review and physical examinations across diverse age groups, from pediatrics to geriatrics.
- Diagnose and treat acute and chronic illnesses, utilizing evidence-based practices and current medical guidelines.
- Manage medication administration, including prescribing, monitoring, and adjusting treatments as necessary.
- Coordinate patient care plans with specialists, hospitals, home care providers, and other healthcare professionals to ensure seamless treatment continuity.
- Utilize electronic health record systems such as Epic and Cerner for accurate documentation, data collection, and medical record management.
- Provide patient education on health maintenance, disease prevention, pain management strategies, and lifestyle modifications.
- Participate in research initiatives or quality improvement projects to enhance clinical outcomes and patient satisfaction.
- Assist in home care visits when required to support patients with mobility issues or chronic conditions.
*Experience*
- Prior hospital experience is highly desirable to handle diverse clinical scenarios efficiently.
- Proven experience in acute care settings with the ability to manage urgent medical situations confidently.
- Strong background in pediatrics with a focus on child health and developmental milestones.
- Experience in pain management techniques to address acute and chronic discomfort effectively.
- Familiarity with medical terminology and proficiency in using electronic health records such as Epic or Cerner systems.
- Demonstrated ability in patient care delivery within a medical office environment or community clinics.
- Knowledge of data collection methods for research or quality improvement purposes is a plus.
- Experience providing home care services to support patients outside of clinical settings is advantageous.
