Outpatient Pharmacist
Job Summary: The Pharmacist is responsible for reviewing prescriptions, dispensing medications accurately, ensuring patient safety, and supporting clinical teams according to hospital policies and standards.
Key Responsibilities: Medication Management Review prescriptions for accuracy and appropriateness
Dispense medications safely and efficiently
Ensure correct dosage, route, and frequency
Monitor drug availability and stock levels
Patient Safety & Compliance Ensure compliance with hospital policies and MOH regulations
Identify potential drug interactions and contraindications
Report medication errors when applicable
Patient Counseling (especially Outpatient) Educate patients on medication use
Explain dosage instructions and possible side effects
Improve medication adherence
Inventory & Documentation Maintain accurate medication records
Monitor expiry dates and storage conditions
Support stock control processes
Operational Duties Work effectively within rotational shifts
Coordinate with medical and administrative teams
Follow infection control and safety standards
Qualifications & Requirements: Bachelor’s Degree in Pharmacy
Valid professional license
1–2 years of hospital experience preferred
Strong clinical knowledge
Good communication skills
Ability to work rotational shifts
