Job Summary
Manage complex social and care needs of nursing home residents by leading a multidisciplinary team to deliver safe, high-quality, and cost-effective care while ensuring compliance with nursing standards and promoting continuous quality improvement.
Responsibilities
- Lead and coordinate a multidisciplinary team including SRNs, ENs, NAs, HCAs, HAs, and Allied Health Professionals to deliver safe and efficient care to residents and their families.
- Manage the increasingly complex social and care needs of nursing home residents to ensure holistic support.
- Oversee compliance with Ministry of Health and Singapore Nursing Board nursing standards to maintain regulatory adherence.
- Plan, conduct, and follow up on quality assurance audits to monitor and improve nursing care quality and appropriateness.
- Promote and implement evidence-based nursing practices to enhance care delivery outcomes.
- Maintain and update written policies, procedures, and work processes to guide nursing care delivery effectively.
- Facilitate smooth transitions for residents from acute to chronic care and back to the community when possible.
- Ensure care delivery aligns with established standards through direct oversight and guidance.
- Assess and ensure appropriate skills mix and manpower resourcing across all shifts and service types.
- Direct staff development by organizing skills and competency upgrading through formal education, in-house training, and approved online learning programs.
- Guide orientation, induction, and preceptorship programs for all new staff to support effective integration and competency development.
- Perform additional duties as assigned by management to support organizational goals.
QUALIFICATION/ EXPERIENCE
- Must have a valid Practicing Certificate with Singapore Nursing Board.
- At least 10 years registered nurse experience, of which 5 years in a nursing home, geriatric or palliative care setting.
- Has either of the following:
- Advanced Diploma in Nursing (Medical-Surgical)
- Advanced Diploma in Nursing (Community Health)
- Advanced Diploma in Nursing (Palliative Care)
- Advanced Diploma in Nursing (Gerontology)
- Advanced Diploma in Nursing (Chronic Disease Management)
- Other post-graduate qualifications in nursing (e.g. Masters in Nursing)
- Able to interact effectively at all levels of staff.
