What You’ll Do

Provide Patient & Family Support

Assess patients’ psychosocial needs related to illness, environment, and support systems

Develop and implement individualized care plans focused on emotional, social, and practical needs

Provide counseling, guidance, and support to patients and caregivers

Assist patients and families with navigating resources, financial concerns, and care planning

Connect Patients to Resources

Coordinate referrals to community resources and support services

Serve as a liaison between patients, families, and community agencies

Build and maintain relationships with local organizations to support patient needs

Collaborate with the Care Team

Partner with nurses, physicians, and interdisciplinary team members to ensure holistic care

Participate in case conferences and contribute to comprehensive care planning

Support discharge planning to ensure safe and successful transitions

Document & Communicate

Maintain accurate, timely documentation in the electronic medical record (EMR)

Communicate patient needs and updates clearly with the care team and referring providers

Uphold professional standards and regulatory requirements

What You Bring

Master’s degree in Social Work (MSW) from an accredited program

Active licensure in the state of practice (as required)

1+ year of healthcare experience preferred (home health strongly preferred)

Current CPR certification

Valid driver’s license, reliable transportation, and auto insurance
