Role Overview The Support Worker is responsible for delivering person-centered care and support that promotes independence, dignity, and well-being. The role involves assisting clients with daily living activities, encouraging participation in the community, and helping individuals achieve their personal goals while ensuring their safety and comfort. Key Responsibilities Provide personal care, including assistance with bathing, dressing, grooming, and hygiene (where required).

Support clients with daily living activities such as meal preparation, shopping, housekeeping, and medication reminders.

Encourage independence by helping clients develop life skills and confidence.

Assist clients in accessing community activities, appointments, education, or employment opportunities.

Offer emotional support and build positive, professional relationships with clients.

Monitor clients' health and well-being, reporting concerns or changes to supervisors.

Maintain accurate records, care notes, and incident reports.

Follow individual care plans and contribute to their ongoing review.

Comply with safeguarding, health and safety, confidentiality, and organizational policies.

Work collaboratively with families, healthcare professionals, and multidisciplinary teams.

Skills & Qualifications Compassionate, patient, and empathetic approach.

Excellent communication and interpersonal skills.

Ability to work independently and as part of a team.

Strong organizational and time management skills.

Basic computer literacy for record keeping.

Relevant qualification in Health & Social Care (or willingness to obtain one).

Previous experience in a care or support setting is desirable but not always essential.

First Aid and medication administration training are advantageous.
