Responsibilities Conduct health screenings and basic patient assessments. Educate patients on chronic disease management, healthy lifestyles and preventive care. Perform blood pressure, glucose, vision, mental health and other basic assessments. Arrange referrals, appointments, follow-ups and investigations. Coordinate patient care with the healthcare team. Provide telephone follow-up and patient support. Maintain accurate nursing documentation and clinical records. Follow patient safety, infection control and clinic policies. Participate in health promotion and quality improvement activities. Attend training to maintain professional knowledge and skills. Maintain clinic cleanliness, equipment and supplies. Perform other duties assigned by the supervisor. Requirement : Polytechnic Diploma in Health Management Services and Health Promotion Good communication and interpersonal skills.
