What You’ll Do
Work under the direction of a Licensed Occupational Therapist to deliver personalized, goal-driven care:
Implement treatment plans to improve patients’ ability to perform daily activities
Provide hands-on therapy focused on strength, coordination, mobility, and self-care
Retrain patients in ADLs (dressing, bathing, meal prep, etc.)
Assess and report on patient progress, function, and home safety needs
Educate patients and caregivers on techniques, exercises, and safety strategies
Identify changes in condition and communicate proactively with the OT and care team
Complete timely, accurate documentation in the EMR
Support a safe, effective, and coordinated plan of care
What We’re Looking For
We’re seeking COTAs who understand the independence and clinical awareness required in home health:
Graduate of an accredited Occupational Therapy Assistant program
Active COTA license/certification in the state of practice
Home Health experience strongly preferred
Current CPR certification
Ability to work independently in a field-based environment
Strong clinical observation, communication, and time management skills
Valid driver’s license, reliable transportation, and auto insurance
