Salary & Benefits
- 💰 Up to $4,000 per month
- 📍 East Singapore
- 🗓️ Monday to Friday
- ⏰ 8:30 AM – 5:30 PM
- 💵 Fixed Monthly Community Care Nursing Allowance
Key Responsibilities
- Provide palliative nursing care for patients
- Perform comprehensive nursing assessments and develop individualized care plans
- Administer and adjust palliative medications under the direction of doctors to manage symptoms effectively
- Work closely with the Medical Director, doctors, and multidisciplinary healthcare teams
- Educate patients, caregivers, and family members on nursing care and symptom management
- Coordinate referrals to inpatient hospices and other healthcare or community support services when required
- Identify patients and families requiring financial assistance or counselling support and facilitate appropriate referrals
- Provide emotional, psychosocial, and bereavement support to patients and their loved ones
- Complete a minimum of six home visits daily
- Participate in community outreach and fundraising activities when required
Requirements
- Must be registered with the Singapore Nursing Board (SNB)
- Diploma or Degree in Nursing or equivalent
- Minimum 1 year of relevant nursing experience
- Experience in palliative care, hospice, home care, or end-of-life care is highly preferred
- Experience with symptom management and titration of palliative medications (e.g. morphine, fentanyl) will be an advantage
- Compassionate, empathetic, and committed to delivering patient-centred care
- Strong communication and interpersonal skills
- Comfortable travelling for home visits
