Social Worker/Senior Social Worker

Salary: Open for discussion (commensurate with experience) Working Hours: 5 Day Work Week Location: Central/West Employment Type: Full-Time

A leading healthcare and community care organisation is seeking Social Workers and Senior Social Workers to join its growing team, providing psychosocial support, counselling and case management services to seniors and individuals with healthcare and social needs.

Job Responsibilities:

• Conduct psychosocial assessments and develop person-centred care plans for clients and caregivers
• Provide counselling, emotional support, crisis intervention and practical assistance to clients and families
• Coordinate referrals and work closely with hospitals, healthcare providers, government agencies and community partners
• Support clients and caregivers in accessing healthcare, social and financial assistance resources
• Collaborate with multidisciplinary teams including nurses, doctors and allied health professionals to deliver holistic care
• Maintain accurate case documentation and prepare reports as required
• Contribute to service improvement initiatives and professional development activities
• Provide guidance and mentoring support to junior staff (for Senior Social Worker level)

Job Requirements:

• Min. Post Graduate Diploma qualification in Social Work recognised by the Singapore Association of Social Workers (SASW)
• Registered Social Worker (RSW) or eligible for registration
• Min. 2 - 5 years of relevant social work experience
• Experience in healthcare, eldercare, community care or case management settings will be an advantage
• Able to work independently and collaboratively within a multidisciplinary environment
• Compassionate, organised and client-centred approach
