Assist licensed nurses in providing direct patient care, including vital sign assessments, medication administration, and wound care

Maintain a clean and safe environment for residents, including assisting with laundry, meal preparation, andhousekeeping tasks

Develop and implement individualized care plans to meet the unique needs of each resident

Collaborate with the healthcare team to identify and address resident needs, concerns, and preferences

Provide emotional support and companionship to residents, fostering a sense of community and belonging

Maintain accurate and up-to-date records of resident care and progress

Requirements:

High school diploma or equivalent required

Current certification as a Nursing Assistant (CNA) in the state of Illinois
