Medical Social Worker

Provide psychosocial assessment and support to patients and their families to address social, emotional and financial needs. Assess patients' social circumstances and identify appropriate support services and interventions. Coordinate with healthcare professionals and community agencies to facilitate holistic patient care. Provide counselling, case management and crisis intervention where appropriate. Assist patients and families with financial assistance, healthcare subsidies and relevant social support schemes. Facilitate discharge planning and referrals to community care and support services. Maintain accurate and confidential case records and documentation. Participate in multidisciplinary team discussions and other departmental programmes and initiatives

Requirements Degree or Graduate Diploma in Social Work

Registered with the Singapore Association of Social Workers

Passion to serve and provide good patient care

Good interpersonal skills and able to work independently
