Our client, a Welsh Council, is seeking an Adults Social Worker to join their Older People Team.

Responsibilities of this role:

  • Undertake strengths-based assessments for older adults, identifying care and support needs, risks, and desired outcomes in accordance with the Care Act 2014.
  • Manage a caseload of older people with a range of physical health, cognitive impairment, frailty, and age-related support needs.
  • Complete Mental Capacity Assessments and Best Interest decisions where appropriate, ensuring compliance with the Mental Capacity Act 2005.
  • Safeguard vulnerable adults by undertaking safeguarding enquiries, assessing risk, and coordinating protective interventions where abuse or neglect is identified.
  • Work collaboratively with health professionals, families, carers, care providers, and community services to develop and review effective support plans.
  • Arrange, monitor, and review care packages and commissioned services to ensure needs are met and independence is promoted.
  • Maintain accurate case records, reports, and assessments, ensuring all interventions comply with statutory duties, legislation, and local authority policies.

Requirements of this role:

  • Must be a Qualified Social Worker, registered with Social Work England.
  • Must have minimum of 3 years post qualifying social work experience
