Palliative Staff Nurse (East | Homecare) #HJO

Job Responsibilities:

• Conduct home visits to provide nursing care for patients with chronic illnesses and palliative care needs.
• Perform comprehensive nursing assessments and develop appropriate care plans.
• Coordinate with doctors on patients' clinical conditions, medical assessments, and follow-up care.
• Administer and monitor prescribed medications, ensuring compliance with treatment plans.
• Monitor patients' progress and implement appropriate nursing interventions.
• Educate patients, caregivers, and family members on home nursing care and disease management.
• Maintain accurate clinical documentation and update patient records using electronic systems.
• Collaborate with the multidisciplinary healthcare team to ensure continuity and quality of patient care.
• Perform other nursing duties as assigned.

Requirements:

• Registered Nurse with a valid Singapore Nursing Board (SNB) practising certificate.
• Palliative care training and experience in home care or community nursing.
• Competent in performing clinical nursing procedures independently.
