Job Description
The Home-Based Physical Therapist is responsible for providing professional physical therapy and rehabilitation services to a patient in a private home setting. The therapist will assess the patient’s physical condition, develop an appropriate treatment program, and provide therapeutic exercises and rehabilitation activities to improve mobility, strength, balance, coordination, and overall functional ability.
Duties and Responsibilities
- Assess the patient’s physical condition, mobility, strength, balance, posture, and functional limitations.
- Develop and implement an individualized physical therapy and rehabilitation program based on the patient’s condition and needs.
- Perform therapeutic exercises, stretching, strengthening, mobility training, and other appropriate rehabilitation techniques.
- Assist the patient with walking, standing, transfers, positioning, and safe movement when necessary.
- Provide balance, coordination, and gait training.
- Use appropriate physical therapy techniques and rehabilitation equipment when required.
- Monitor the patient’s progress and adjust the treatment program accordingly.
- Help prevent muscle weakness, stiffness, contractures, and other complications caused by limited mobility.
- Educate the patient and family members regarding proper exercises, positioning, mobility, and home safety.
- Encourage the patient to participate actively in rehabilitation activities.
- Maintain accurate records of treatment sessions, progress, observations, and recommendations.
- Coordinate with physicians, rehabilitation specialists, nurses, or other healthcare professionals when necessary.
- Ensure the patient’s safety and comfort during all therapy sessions.
- Maintain cleanliness and proper organization of therapy equipment and the patient’s rehabilitation area.
- Follow professional standards, patient confidentiality requirements, and health and safety procedures.
- Perform other rehabilitation-related duties reasonably required based on the patient’s condition.
