Job Title: Social Worker - CMHT - Rochdale Location: Rochdale (Hybrid) Pay Rate: £30 per hour

Job Description:

Liquid Personnel is recruiting Social Worker for it client's Community Mental Health Team (CMHT) located at Rochdale.

What will your responsibilities be?

Undertake Strengths-Based Needs Assessments and review care packages for adults with severe and enduring mental health needs

Update care plans and complete Mental Capacity Assessments where required

Carry out safeguarding work in line with statutory duties when appropriate

Support timely hospital discharges for individuals under secondary mental health services

Work with both in-borough and out-of-borough placements; understanding of mental health pathways is beneficial

Qualifications & Experience:

To be successful in this role you must have,

Social Work England registration

Eligible to work in the UK

Hold a full UK driving licence

Post-qualified experience

Degree level or equivalent in Social Work
