Care Connections Lead Full-Time | $25–$28/hour (DOE) w/ bonus opportunities

We’re looking for a high-energy, results-driven professional to own both member acquisition and caregiver recruiting from start to finish. If you’re someone who can build pipelines, move fast, and connect the right people at the right time—this role is for you.

What You’ll Do

Build and manage a pipeline of member referrals from inquiry to service start.

Recruit, screen, and hire caregivers to support growth demand.

Move referrals and candidates through each stage quickly.

Ensure clean, accurate documentation and onboarding readiness.

Match caregivers to members based on needs, preferences, and schedules.

Track and report on growth metrics weekly.

What We’re Looking For

2+ years in healthcare, recruiting, intake, or care coordination (DDD/ALTCS experience preferred)

Strong communication skills with the ability build trust with families and possible candidates.

Highly organized with strong follow-up and pipeline management skills.

You’re goal oriented and motivated by hitting targets.

Comfortable working across operations, HR, and local office teams.

Bilingual in Spanish and English required.

Schedule & Work Environment

Monday – Friday, 8:00 AM – 5:00 PM (flexible; occasional evenings/weekends)

Fast-paced, data-driven role with high accountability and clear expectations

Responsibilities are a mix of recruiting, onboarding, intake coordination and relationship management.
