Job Title: Social Worker - Older Adults Team Location: Waltham Forest Pay Rate: £33.42 per hour
Job Description:
Liquid Personnel is recruiting experienced Social Worker for its client's Older Adults Team at Waltham Forest. You’ll be working with individuals aged 65+ with a range of needs, including physical frailty, dementia, and long-term conditions.
What will your responsibilities be?
Undertake strengths-based assessments and reviews
Develop and review care and support plans
Work closely with health professionals, families, and community services
Apply the Care Act 2014, Mental Capacity Act, and safeguarding procedures
Promote independence and person-centred outcomes
Use Mosaic / Liquidlogic (or relevant case management system)

Qualifications & Experience:
To be successful in this role you must have,
Social Work England registration
Eligible to work in the UK
Hold a full UK driving licence
Post-qualified experience
Degree level or equivalent in Social Work
