What You'll Do

As a Clinical Coordinator, you'll play a critical role in ensuring our patients receive seamless, compassionate care while supporting our clinical team and daily operations.

Patient Care Coordination

Serve as a key liaison between patients, families, physicians, and hospice clinicians.

Coordinate care services and assist with resolving patient care concerns.

Support communication across the interdisciplinary team.

Clinical Documentation & Orders

Request, track, clarify, and process physician orders.

Upload and manage documentation within the EMR system.

Ensure records are complete, accurate, and compliant.

Assist with clinical documentation and workflow management.

Clinical Operations

Support field clinicians with scheduling and care coordination.

Monitor and maintain medical supply inventory.

Ensure the supply room remains organized and OSHA compliant.

Assist with audits, quality improvement initiatives, and regulatory readiness.

Team Support

Assist with onboarding and orienting new team members.

Collaborate with nurses, social workers, chaplains, physicians, and leadership.

Contribute to a positive and supportive team culture.

Qualifications

Required

Current California LVN License in good standing

Strong communication and customer service skills

Excellent organizational and multitasking abilities

Ability to work independently and prioritize responsibilities

Knowledge of medical terminology

Proficiency with computers and electronic medical records

Preferred

Experience in Hospice, Home Health, Palliative Care, Skilled Nursing, or Case Management

BLS Certification

Associate Degree or higher

Experience coordinating patient care in a healthcare setting
