What You’ll Do

Provide compassionate, hands-on care aligned with each patient’s individualized plan of care:

Assist with personal care (bathing, dressing, grooming, toileting, hygiene)

Support safe mobility (ambulation, transfers, range of motion exercises)

Monitor and report vital signs and patient condition changes

Assist with meals, feeding, and light housekeeping

Support medication self-administration (as directed)

Maintain a clean, safe patient environment

Document care accurately and timely in the EMR

Communicate proactively with nurses and care team members

What We’re Looking For

We’re seeking aides who understand the pace, independence, and impact of home health care:

Active CNA or Home Health Aide certification (in good standing with state registry)

Home Health experience strongly preferred

Current CPR certification

Ability to work independently in a field-based setting

Strong observation and communication skills

Compassion for patients who are ill, aging, or recovering

Reliable transportation, valid driver’s license, and auto insurance
