Occupational Therapist (West, (Community Hospital)

Conduct patient assessments and develop individualised therapy plans based on functional needs and treatment goals. Deliver occupational therapy interventions and provide guidance to patients and caregivers to support recovery and independence. Coordinate discharge planning and monitor patient progress, ensuring appropriate follow-up and continuity of care. Support clinical quality initiatives, including audits, risk management and review of clinical practices and guidelines. Participate in staff training, knowledge sharing and service improvement projects within the therapy team.

Requirements:

• Diploma in Occupational Therapy, recognised by the AHPC Singapore.
• At least 2 years of relevant occupational therapy experience in rehabilitation or hospital settings.
• Strong patient communication, clinical reasoning and problem-solving skills.
• Good interpersonal skills with the ability to work effectively within a multidisciplinary team.
• Responsible, adaptable and committed to delivering quality patient care.
