Social Worker – Oncology / Blood Cancer

Leukemia & Lymphoma Foundation (LLF) is seeking an experienced Social Worker to provide psychosocial support to individuals affected by blood cancer and their caregivers. The role will work closely with healthcare institutions and community partners to support patients across diagnosis, treatment, recovery and survivorship.

Key Responsibilities

Patient & Caregiver Support

• Conduct psychosocial assessments and identify patients’ and caregivers’ emotional, social, financial and practical needs.
• Provide individual counselling, emotional support, case management and follow-up.
• Support patients and families in coping with diagnosis, treatment, discharge and life after cancer.
• Provide caregiver support and facilitate access to appropriate community and social services.
• Identify complex needs and make appropriate referrals to healthcare or community professionals.

Hospital & Community Engagement

• Work closely with medical social workers, doctors, nurses and allied health professionals at LLF’s partner healthcare institutions.
• Provide continuity of psychosocial support as patients transition from hospital to the community.
• Support patients in areas such as return to work or education, family relationships, social reintegration and survivorship.
• Build and maintain effective relationships with healthcare and community partners.

Programmes & Service Development

• Facilitate patient and caregiver support groups, educational sessions and psychosocial programmes.
• Contribute to the development of LLF’s oncology and survivorship support services.
• Identify emerging needs among blood cancer patients and caregivers and propose appropriate programmes or interventions.
• Participate in service evaluation, outcome tracking and reporting.

Case Management & Documentation

• Manage an assigned caseload and ensure appropriate assessment, intervention and follow-up.
• Maintain accurate and timely case documentation in accordance with professional and organisational standards.
• Participate in multidisciplinary discussions and case reviews where appropriate.

Requirements

• Master’s Degree in Social Work from a recognised institution.
• Minimum 3–5 years of relevant social work experience, preferably in healthcare, oncology, haematology, hospice/palliative care or a related setting.
• Strong experience in psychosocial assessment, counselling and case management.
• Good understanding of Singapore’s healthcare and social service landscape.
• Strong interpersonal and communication skills, with the ability to engage patients and caregivers with empathy and professionalism.
• Mature, resilient and able to manage sensitive and emotionally challenging situations.
• Able to work independently while collaborating effectively with healthcare professionals and community partners.
• Experience in programme facilitation, support groups or community-based work will be an advantage.
