What You’ll Do

Coordinate care delivery and support hospice operations through strong scheduling and communication:

Schedule and coordinate patient visits across hospice clinicians to ensure timely, efficient care

Collaborate with nurses, social workers, chaplains, and leadership to support daily care coordination

Monitor schedules and follow up on missed, incomplete, or delayed visits

Utilize EMR and reporting tools to track productivity, coverage, and scheduling optimization

Partner with field teams and leadership to reduce missed visits and improve patient experience

Communicate effectively with patients, families, clinicians, and referral partners

Respond to changing patient needs and scheduling challenges with urgency and problem-solving

Maintain accurate documentation and workflow completion within the EMR system

Support quality initiatives, including participation in QAPI programs

Promote a culture of compassion and professionalism in every interaction

What We’re Looking For

We’re seeking individuals who thrive in a fast-paced, team-oriented healthcare environment:

High school diploma or equivalent required

1+ year of administrative, scheduling, or office experience (healthcare or hospice preferred)

Strong organizational and multitasking skills

Experience working with electronic medical records (EMR) or scheduling systems preferred

Excellent communication and interpersonal skills

Ability to prioritize, adapt, and problem-solve in real time
