The Renfrew Victoria Hospital (RVH) and St. Francis Memorial Hospital (SFMH) are fully accredited facilities; RVH is 45 minutes west of Ottawa and SFMH is in beautiful Barry?s Bay, about an hour west of Renfrew. Together we provide medical, surgical and complex continuing care as well as extensive ambulatory care with over 42,000 emergency and clinic visits per year (30,000 RVH; 12,000 SFMH).

We are seeking an Occupational Therapist Physiotherapist to join our new, innovative ALIGN program (Aging at Home through Linked, Integrated, and Guided Networked Care - A Collaborative Model). This integrated role for both St. Francis Memorial Hospital and Renfrew Victoria Hospital is a unique position that bridges acute care and community-based practice.
The successful candidate will play a key role in facilitating safe and effective transitions from hospital to home by reducing barriers to discharge and supporting recovery in the community. Working collaboratively with patients, caregivers, healthcare providers, and community partners, the clinician will assess functional and physical needs, identify barriers to independence and develop individualized care plans that support successful reintegration into the community.
*Pay Grid: $40.12 - $55.61/hourly*

*Key Responsibilities*
*Shared Responsibilities:*
- Participate in discharge planning and transitional care initiatives.
- Identify barriers to successful discharge and community reintegration.
- Develop individualized care plans based on patient goals and needs.
- Connect patients with community resources and support services.
- Provide education to patients, families and caregivers.
- Monitor progress post-discharge and adjust interventions as appropriate
- Maintain accurate clinical documentation.
- Participate in quality improvement program evaluation activities.
- Provide direct inpatient rehabilitation services to patients at SFMH and deliver assessments and interventions across both ALIGN partner sites, in hospital and at home.
- Review and prioritize OT or PT referrals and consultations of both SFMH inpatients and ALIGN program.
*Occupational Therapist Responsibilities:*
- Assess functional abilities related to activities of daily living.
- Evaluate cognitive, psychosocial and environmental factors impacting independence.
- Recommend adaptive equipment, home modifications etc.
- Conduct home safety assessments and support community reintegration.
- Promote independence and occupational performance in the home and hospital setting.
*Physiotherapist Responsibilities:*
- Assess mobility, balance, strength, endurance, and physical function.
- Develop and implement rehabilitation programs to improve mobility and prevent decline
- Prescribe mobility aids and recommend equipment to enhance safety and independence
- Assess fall risk and implement prevention strategies both at home and in hospital
- Support patients in achieving functional mobility goals within their home and community
