Clinical Team Lead

In this key leadership role, you'll help guide a multidisciplinary team of clinicians, support high-quality patient care, and make a meaningful impact on the lives of patients and families in the comfort of their homes.

What You'll Do

As the Clinical Team Lead, you will:

Provide clinical leadership and support to nurses, aides, therapists, and social workers

Coordinate patient care, clinician assignments, and referrals

Oversee care plans to ensure quality outcomes and regulatory compliance

Review clinical documentation and support timely billing and reimbursement processes

Monitor high-risk patients and collaborate with clinicians to reduce hospitalizations

Conduct chart audits and participate in quality improvement initiatives

Serve as a clinical resource and mentor for field staff

Support staff onboarding, education, and competency development

Step into leadership responsibilities in the absence of the Clinical Director/Manager

What We're Looking For

Active Ohio RN or LPN license

Minimum 3 years of clinical nursing experience

At least 1 year of home health experience required

Previous supervisory or leadership experience preferred

Strong communication, critical thinking, and organizational skills

Valid driver's license and reliable transportation
