Programme Executive, SD (4 months contract) - hybrid arrangement
Conduct case assessment and formulate Individualized Care Plan for persons with dementia and their carers. Plan and facilitate group sessions/ activities for carers and/or persons with dementia. Provide intervention such as psychoeducation, supportive counselling and service linkages. Liaise and collaborate with multidisciplinary professionals and external service providers for holistic care management. Provide timely and accurate case recordings and relevant documentations. Participate in team meetings and case conferences as required. Assist promptly in the preparation of data and statistics for programme evaluation and reporting purposes. Explore new avenues of collaboration with community partners and stakeholders. Support in outreach events and talks. Any ad-hoc tasks as and when required.
Requirements
- Degree in Social work and Psychology, or any relevant discipline, with a minimum of 2 years of experience in community engagement and/or case management.
- Ability to conduct Advance Care Planning conversation or prior experience working with persons with dementia or carers will be an added advantage.
- Requires work on scheduled Saturdays and occasional weekday evenings.
- Good oral, written and interpersonal communication skills.
- Ability to facilitate group sessions will be an added advantage.
- Self-motivated with a high level of initiative.
- Resourceful and adaptable.
- Strong attention to detail.
- Good organizing and execution skills.
