Hospital Pharmacist (AN)
Review prescriptions and medication charts to ensure medicines, doses and directions are appropriate and clearly prescribed.
Identify potential medication-related issues, including drug interactions and adverse reactions, and communicate with the prescribing doctor when clarification or intervention is required.
Accurately process and dispense medication orders within the required timeframe.
Verify prescriptions and provide patients with appropriate medication counselling and information.
Coordinate and oversee medication-related activities for inpatient and outpatient discharges.
Provide drug information and medication-related support to doctors and other medical staff.
Prepare extemporaneous pharmaceutical products when required and prescribed.
Maintain accurate records of pharmaceutical inventory and conduct regular stock checks across wards.
Monitor medicine stock levels and arrange replenishment to maintain adequate supplies.
Supervise and guide Pharmacy Assistants and Pharmacy Technicians in their assigned duties.
Communicate with external doctors and Resident Medical Officers when necessary regarding medication orders and related matters.
Ensure pharmaceutical products are received, stored, supplied and distributed in accordance with applicable legal and hospital requirements.
Maintain all required documentation and records relating to medication management and supply.
