Care Services Coordinator (AN)

Develop and review individualised care plans for residents based on their identified care and support needs, in accordance with the Home’s policies and procedures.

Implement the agreed care plans and monitor residents’ progress, documenting relevant observations and follow-up actions.

Plan, coordinate and implement appropriate programmes and activities to support residents’ care needs, well-being and quality of life.

Prepare six-monthly review reports and maintain monthly interview records for approximately 50 residents. We regret only shortlisted candidates will be contacted.
