Clinical Nurse Lead / Senior Community Nurse

Deliver professional nursing care to patients in the community, ensuring care is safe, effective, and aligned with clinical best practices.

Guide, coach, and mentor nursing staff to strengthen clinical skills, confidence, and service quality.

Review and assess new patient referrals, supporting care planning and ensuring appropriate service allocation.

Collaborate with multidisciplinary healthcare professionals to coordinate holistic patient care and achieve positive patient outcomes.

Provide clinical advice and support to nursing team members, assisting with complex cases and escalation management.

Monitor nursing operations, staffing allocation, and resource utilization to maintain efficient daily service delivery.

Contribute to the development, review, and enhancement of clinical guidelines, nursing protocols, and standard operating procedures.

Support staff development through coaching, competency assessments, performance feedback, and ongoing learning initiatives.

Participate in quality improvement projects, clinical audits, and patient safety initiatives to continuously improve healthcare services.

Attend team meetings and support operational planning, administrative tasks, and departmental initiatives.

Requirements: Registered Nurse with a valid Singapore Nursing Board (SNB) registration.

Relevant experience in community healthcare, home care, or related nursing environments will be an advantage.

Strong leadership, mentoring, and clinical decision-making skills.

Able to work independently while collaborating effectively with multidisciplinary teams.

Good communication, interpersonal, and organizational skills.

Passionate about delivering patient-centred care and driving continuous quality improvement.
