Prior exp with medical social work in hospital setting is required

Our client is a leading Healthcare provider

Responsibilities:

  • Provide and facilitate socio-economic evaluations to help patients and their families address practical concerns, such as financial matters, which are often an issue at the end of life.
  • On-going individual and family counselling through face-to-face sessions or home visitations in order to help navigate the personal and ethical dilemma, cultural conflict and issue relating to religion, spirituality and the meaning of life.
  • Work with the broader community which includes educational, religious, and social service organizations so that patients and their families can achieve optimal end of life goals.
  • Engage families’ involvement and encourages their visits, as well as prepares patients / families for the end-of-life stage and managing the life closure and the dying process.
  • Provide bereavement information, education and support to help the families of the deceased patients cope with their grief and loss.
  • Attend Interdisciplinary Disciplinary Meetings with medical, nursing, allied health colleagues to discuss plans for total management of the patient.
  • Counsel patients and their family members on financial and discharge care plans.
  • Perform field work involving visiting patients in hospital wards and other social agencies to help patients alleviate or resolve their medico-social problems.
  • Prepare social and other related reports for management and other related institutions / agencies.
  • Present cases for discussion and engages actively on formal platforms such as departmental or inter-departmental meetings for the purpose of sharing, reviewing and clarifying of atypical, difficult issues.
  • Maintain constant networking with counterparts, Counsellor, staff nurse, nursing officer and relevant medical staff / authorities of other organisations.
  • Undertake more workload or complex cases when required.
  • Provide formal clinical supervision to juniors and/or student interns and provides case consultations.
  • Vet reports and important documentation prepared by supervisees before submission to CEO/Committees for final approval.
  • Orientating and OJT (on-the-job) Training to new staff and/or attached interns on their performance of duties.
  • Design and teach on programmes within organisation, for instance but not limited to journal club, department learning, inter-departmental training etc.
  • Teach on external programs.
  • Lead small-scale research that is related to and in line with the overall organisation's research strategy and/or QI project on improving work related processes.
  • Undertake management of complex cases.
  • Document all intake assessments, progress notes and social interventions for record, within the expected time frame set by the department.
  • Assist Team Leads and team members to review work processes on regular basis.
  • Undertake to keep oneself well informed of the updated industry guidelines and develops new skills that are necessary for the job.
  • Maintain high standards of housekeeping and workplace organisation at all times.
  • Any other duties as and when assigned.

Requirements:

  • Degree in Social Work/ Counselling and / or related Masters in Social Work / Counselling or related fields.
  • Registered with the Singapore Association of Social Workers for Social Workers or Singapore Association for Counselling (SAC).
  • At least 5 to 8 years of medical social work experience in a healthcare environment preferably.
  • Bilingual and ability to communicate in local dialects preferably.
  • Possess knowledge and understanding of legislative policy and practice/research issue in the field of palliative care.

Other Information:

Working Days/Hours: Mon to Thurs (8am to 530pm), Fri ends at 5pm

Location: Near Marymount MRT (Transport provided at MRT)

Up to S$6.8K Basic+allowances+bonuses!
