We are seeking a passionate and experienced Social Worker to join our team. Reporting to the Head of Client Services , you will be responsible for providing professional social work services to persons with visual impairment and their families through casework, counselling, group work, advocacy and community resource coordination. The Social Worker adopts a client-centred and strengths-based approach to support clients in achieving their rehabilitation goals, enhancing psychosocial well-being, and promoting independent living and community integration. Responsibilities

  1. Client Assessment & Case Management

Conduct comprehensive intake assessments and interviews with clients, family members, and caregivers, including registration for SAVH services

Assess clients’ psychosocial, financial, family and support needs, and develop appropriate intervention plans

Develop, implement and review set goals with clients and relevant stakeholders measurable outcomes

Provide case management and follow-up services to support clients’ rehabilitation and adjustment to vision loss

Conduct home visits where required to better assess client’s living conditions and support needs

Maintain accurate, timely, and comprehensive case records, case notes, assessments, and related documentation in accordance with organisational policies and professional standards

  1. Service Coordination & Collaboration

Collaborate and participate in case discussion with internal departments and multidisciplinary teams to ensure coordinated and client-centred service delivery

Liaise with healthcare institutions, social service agencies and community partners on referrals, rehabilitation, and support services

Work with relevant stakeholders to facilitate clients’ access to appropriate community resources, financial assistance and social support

Support the coordination and utilisation of Low Vision Devices (LVDs) through the Assistive Device Centre (ADC), including facilitating communication among clients, healthcare providers, and relevant stakeholders.

Encourage and support client participation in the Vision Rehabilitation Programme (VRP) to promote engagement and rehabilitation outcomes

  1. Counselling and Intervention

Provide counselling, emotional support, appropriate psychosocial interventions to clients and their families

Support clients in adjusting to vision loss and developing coping strategies, resilience and self-advocacy skills

Provide support in areas such as family relationships, financial difficulties, social isolation, emotional well-being and independent living

Identify and respond appropriately to clients who may require additional or specialised support, including making referrals to relevant services where appropriate

Support clients and families during period of crisis or significant life transitions, with appropriate consultation and supervision where required

  1. Community Engagement & Programme Development

Participate in outreach, engagement, and Association events to increase awareness of Association’s services available to persons with visual impairment

Contribute to and support community initiatives that promote inclusion, accessibility and participation of persons with visual impairment

Gather feedback from clients and stakeholders to identify service needs and opportunities for improvement

  1. Monitoring, Reporting & Professional Practice

Monitor and evaluate client progress against intervention goals

Prepare case reports, programme statistics and other documentation required for service monitoring, evaluation, and funding requirements, including submissions via the EPES system

Participate actively in case discussions, team meetings, supervision, training, and professional development to enhance service delivery and maintain professional competency

Maintain professional boundaries, confidentiality and ethical standards in all aspects of practice

Keep abreast of relevant developments in social work practice, disability services and rehabilitation

  1. Advanced Practice Responsibilities

Promote a culture of learning and continuous improvement by sharing relevant knowledge, best practices, research insights, and new approaches with the team

Undertake additional duties and responsibilities as assigned by the Head of Department (HOD)

Requirements Degree in Social Work from a recognised institution

Accredited or eligible for accreditation with the Singapore Association of Social Workers (SASW)

Minimum 3 years of relevant experience , preferably in healthcare, disability, or rehabilitation settings. Candidate with more years of working experience may be considered for Senior Social Worker

Strong competencies in case management, psychosocial assessment, and counselling

Experience working in multidisciplinary teams and collaborating with community partners

Experience in group work facilitation and/or programme development will be an advantage

Relevant training in counselling approaches or case management is preferred

Strong interpersonal and communication skills, with the ability to engage clients from diverse backgrounds

Ability to manage complex cases and effectively address resistance to change

Good organisational and documentation skills, with familiarity in reporting systems (e.g., EPES) an advantage

Strong critical thinking and problem-solving abilities

Compassionate, client-centred, and committed to professional ethics and standards

Proactive, adaptable, and able to work both independently and as part of a team

Demonstrated ability or readiness to mentoring / supervision
