SAMH SAY-IT! (Sports, Arts, Youth Integrated Team) provides assessment and intervention for youths and engages youths for ongoing monitoring and support. YIT also works actively with youth agencies, schools and community partners to provide holistic case management for youths and their families. Job Purpose The Case Worker/Social Worker supports Head in overseeing the YIT team to ensure service delivery is in accordance to the intended programme objectives and care model for YIT programme as outlined under the terms & conditions of the Letter of Award/Addendum. This role requires the identified personnel to have the relevant competencies, with sufficient years of direct work experience and service with clients with mental health issues, necessary for the purpose of supervision of staff in the respective field. He/She assists in maintaining an overview of the service planning and implementation of the YIT programme. He/She will work closely and provide guidance to the YIT team to achieve the YIT goals of outreach to increase mental health awareness, promote early identification and link those at risk and with needs to the appropriate support and resources, and provide follow up support. In addition, the Case Worker/Social Worker will establish and/or maintain networking with other community partners or agencies, for purpose of collaboration and facilitate care coordination. Job Responsibilities A). Case Management Conduct comprehensive biopsychosocial assessments to understand the needs, risks and strengths of youth clients and their families. Develop and implement individualised care plans in collaboration with clients, caregivers and multidisciplinary team members. Provide evidence-based psychosocial interventions including counselling, psychoeducation, safety planning and family work. Monitor progress, evaluate effectiveness of interventions and adjust care plans accordingly. Coordinate care and maintain close communication with schools, healthcare professionals, community partners and relevant agencies. Participate in multidisciplinary case discussions and contribute to care planning and reviews. Assist in managing a caseload of youth clients under supervision of Supervisor. Support the full case management cycle: from intake, needs assessment, care planning, service coordination to discharge planning. Conduct regular check-ins with clients and caregivers to monitor progress and wellbeing. Liaise with external agencies (schools, hospitals, family service centres etc.) to facilitate referrals, joint sessions and care coordination Provide emotional support and practical guidance to youth clients under supervision. Build rapport and foster trust to enhance client engagement and motivation. Help coordinate case conferences and multi-disciplinary meetings. Identify presenting/underlying issues to design a care plan to enhance/maintain and promote psychosocial functioning of clients. Link youths or caregivers with appropriate services for continuity of care. Conduct risk assessments and ensure safety plans. Administer K-10, PHQ-4, GAD-7 and PHQ-9, CSSR-S screening tools and/or other tools necessary for youths and ensure proper documentation. Identify and assess risks related to self-harm, suicide, abuse or neglect. Develop and implement safety plans where necessary, in consultation with the client, caregivers and team. B). Administration and Documentation Tasks

Maintain accurate and timely documentation in accordance with case management protocols. Ensure accurate and confidential documentation of client records and administrative files. Prepare simple case summaries, progress reports/case notes and administrative documents. Ensure data is documented in both eCMS and AIC Template. Adhere to Programme’s key performance indicators (KPIs). C). Therapeutic and Group Interventions Design and facilitate group-based interventions that promote emotional regulation, resilience, social skills, and recovery. Support the planning, implementation, and evaluation of youth mental health programs, workshops, and outreach initiatives. Engage youths through activities that promote emotional regulation, self-awareness, and coping skills. Work alongside Psychologists and Social Workers to deliver integrated care. D). Professional Development Attend Individual and Group supervisions, team meetings, and training sessions to deepen knowledge in youth mental health. Participate in case discussions, supervision and team meetings to contribute observations and insights. E). Professional Development To assist Head to organise recreational activities for the team when necessary. To assist on ad-hoc SAMH projects, community outreach programmes and events, ie World Mental Health Day, YOUth Alive!, etc. assigned by Head. Any other duties as assigned by Head. F). Operations Taking ownership of the centre. Maintenance of premises. Qualifications and Experience Minimum Degree in Psychology, Social Work or related discipline. Minimum 1 year of relevant work experience is preferred. Experience working in Social Service Agencies especially youth work, will be an added advantage. Experience with facilitation, case management and counselling. Proficient in MS Word, Excel, Powerpoint and database management. Strong written and verbal communication skills. Strong organisational and time management skills, with attention to details.
