Leaving Care Support Worker

We are seeking a dedicated Leaving Care Support Worker to join our Children’s Services team. This role is crucial in assisting young people aged 16-25 transitioning to adulthood, particularly those leaving care or facing homelessness. The successful candidate will work within a supportive team environment to help these young individuals build their futures through tailored support and guidance.

Day-to-day of the role:

• Act as a Personal Advisor to young people, providing crisis and long-term support.
• Manage individual caseloads, liaising with Pathway Coordinators, social workers, and other service providers.
• Prepare, produce, implement, and evaluate plans with individuals, families, and professional colleagues.
• Assess needs and recommend courses of action to support young people in achieving positive outcomes.
• Contribute to the planning and development of team and departmental services.
• Maintain accurate records and participate in decision-making forums.
• Work collaboratively with partner agencies to provide comprehensive support services.

Required Skills & Qualifications:

• Knowledge of issues facing young people leaving care or becoming homeless.
• Understanding of relevant childcare legislation and policy.
• Proficient in counselling, welfare rights, and IT support skills.
• Excellent communication skills, both verbal and written.
• Ability to undertake assessments and review programmes, including pathway plans.
• Demonstrated organisational skills with the ability to meet deadlines.
• NVQ Level 3 Caring for Children and Young People or equivalent qualification.
• Experience working with young people in a similar setting is highly desirable.
