Adults Social Worker [Trafford]

Our Client, a Greater Manchester council is seeking an Adult’s Social Worker to join their Interim Triage Team

Responsibilities of this role:

• Undertake initial screening and triage of referrals to Adult Social Care, identifying risks, needs, and appropriate service pathways.
• Complete timely assessments and gather relevant information to determine eligibility and support requirements under the Care Act 2014.
• Manage urgent and complex enquiries, ensuring individuals receive the right support at the earliest opportunity.
• Provide information, advice, and guidance to adults, carers, and professionals to promote wellbeing and independence.
• Work collaboratively with health services, community organisations, and partner agencies to coordinate effective interventions and prevent escalation of need.
• Complete safeguarding referrals, risk assessments, and appropriate actions to protect vulnerable adults from abuse and neglect.
• Maintain accurate case records, assessments, and reports, ensuring compliance with legislation, policies, and performance standards.

Requirements of this role:

• Must be a Qualified Social Worker, registered with Social Work England.
• Must have minimum of 3 years post qualifying social work experience
