Adults Social Worker - Continuing Healthcare

Our Client, Blackpool Council, is seeking an Adult’s Social Worker to join their Team.

Responsibilities of this role:

• Complete comprehensive social care assessments to support NHS Continuing Healthcare eligibility decisions.
• Contribute to multidisciplinary team meetings and provide professional social work recommendations regarding CHC funding.
• Assess mental capacity and apply the principles of the Mental Capacity Act when making best interest decisions.
• Advocate for individuals and families throughout the CHC assessment, review, and appeal processes.
• Work collaboratively with health professionals, providers, family members, and other agencies to coordinate care and support planning.
• Undertake safeguarding enquiries and manage risks to ensure the safety and wellbeing of vulnerable adults.
• Support hospital discharges by facilitating timely assessments and arranging appropriate community care packages.
• Review existing care arrangements to ensure needs are met and funding remains appropriate.
• Maintain accurate case records, reports, and assessments in line with legislative and organisational requirements.
• Provide guidance and advice to individuals, families, and professionals regarding CHC processes, eligibility criteria, and available support services.
• Manage a complex caseload, prioritising work effectively to meet statutory duties and timescales.

Requirements of this role:

• Must be a Qualified Social Worker, registered with Social Work England.
• Must have a Social Work qualification (Diploma in social work, social work degree or equivalent)
• Must have minimum of 3 years post qualifying experience working as a social worker
