Adult Social Worker- Various Teams

Our Client, a Yorkshire council is seeking an Adult’s Social Worker to join one of their various Adult's Team.

Responsibilities of this role:

• Undertake strengths-based assessments, reviews, and care planning for adults with a range of complex needs in line with the Care Act 2014.
• Manage a diverse caseload, promoting independence, wellbeing, and positive outcomes through person-centred practice.
• Complete safeguarding enquiries and interventions to protect adults at risk, ensuring effective risk assessment and management.
• Undertake Mental Capacity Assessments and Best Interest decisions in accordance with the Mental Capacity Act 2005.
• Work collaboratively with health colleagues, partner agencies, families, carers, and community organisations to coordinate support and achieve positive outcomes.
• Facilitate timely discharges, transitions, and access to appropriate services, resources, and community support networks.
• Maintain accurate records, assessments, reports, and support plans, ensuring compliance with statutory duties, policies, and professional standards.

Requirements of this role:

• Must be a Qualified Social Worker, registered with Social Work England.
• Must have minimum of 3 years post qualifying social work experience
