Virtual Assistant Care Coordinator | Healthcare 📍 Jurong 💰 Up to $3,500/month 📅 8-Month Contract Working Hours • Monday–Thursday: 8.30am–6.00pm • Friday: 8.30am–5.30pm

Patient Outreach & Virtual Care

• Conduct outreach to eligible patients and support programme enrolment • Monitor the patient dashboard daily and review responses received through the patient-facing chatbot • Route and escalate patient responses to the appropriate clinical team according to established workflows and protocols • Follow up with non-responsive patients and guide them on using the chatbot • Verify patient enrolment and ensure information is accurately reflected on the dashboard • Communicate professionally with patients and caregivers through telephone and digital channels

Care Coordination

• Coordinate appointments, referrals, handovers and communication between patients and clinical teams • Recognise potential red flags and escalate concerns promptly according to established protocols • Maintain accurate and timely documentation of patient interactions, outcomes and escalations across relevant IT systems • Follow up on cases and ensure required actions are completed

Data, Reporting & Improvement

• Maintain and update patient databases accurately • Collate and validate data relating to enrolment, response rates, outreach attempts, escalations and patient outcomes • Analyse data to identify trends, care gaps and recurring non-response patterns • Prepare regular operational reports and updates • Support audits, service evaluation and continuous improvement initiatives • Use data to identify opportunities to improve workflows and patient engagement
