Temp Patient Care Coordinator - Virtual Assistant (Healthcare) #HZO

Duration: 8 months Location: West Working Hours: 5 days office hours Job Scope: Conduct patient outreach and enrolment, ensuring eligible patients are accurately registered. Monitor patient responses via chatbot dashboards and follow up with non-responsive patients. Coordinate appointments, referrals, handovers and communication between patients and clinical teams. Maintain accurate patient databases, interaction records and documentation across IT systems. Analyse enrolment data, response rates and patient outcomes, prepare reports, and support audits and process improvements. Job Requirements: Min. Diploma in Health Sciences or any relevant field. Customer service or healthcare experience will be an added advantage.
