Salary: up to $5000 Working Days & Hours: • Mon - Wed 8.30 to 5.15
- Thurs - Fri, 8.30 to 5.30
- One Saturday in every 4 weeks, 8.30 to 12.30pm
Location: Novena Job Description: * Review transfer medications appropriately, contact patients pre-transfer on medication supply needed, and answer medicine related questions
- Medication counselling on transfer medications by a variety of means (in-person and virtual)
- Medication reconciliation
- Review discharge prescriptions appropriately
- Verify new orders by the physicians and identify inappropriate medication/doses if relevant
- Assist with perform medication supply duty
- Follow up on medication preparation status
- Undertake duties which may be assigned by the staff member depending on the requirements
