PERM Homecare Staff Nurse #HJN

Duties: Provide nursing care to clients in home and community settings, including wound dressing, medication, catheter, stoma and tube feeding care. Monitor clients’ conditions, administer treatments and medications, and promptly report any changes or clinical concerns. Conduct nursing assessments and support the development and review of individualised care plans. Coordinate care with doctors, therapists, caregivers and healthcare teams, including discharge and long-term care arrangements. Maintain accurate clinical documentation and ensure compliance with nursing standards, infection control, patient safety and regulatory requirements. Educate and support clients, families and caregivers while working closely with Healthcare Assistants and Operations teams. Working hours: 5 days (Mon-Fri), Sat only when needed.
