Part-Time Patient Liaison Assistant (Healthcare) #HZO

Location: West Salary: $18/ hr Working Hours: Weekdays 8am - 2pm, Weekends 8am - 11am

Job Scope:

• Engage patients and caregivers through counselling and outreach
• Explain programme requirements and care processes clearly
• Verify patient contact and residential information
• Address patient and caregiver concerns and facilitate programme admission
• Escalate complex cases or concerns to the clinical team when required
• Work closely with intake colleagues to ensure a smooth transition into the service
• Maintain accurate records of counselling sessions and outcomes
• Support process improvements and other ad hoc duties

Job Requirements:

• Min. Diploma in any healthcare/ any relevant field
• Able to commit to weekday shifts and rotational weekends/Public Holidays
• Strong communication and interpersonal skills
• Empathetic and patient-centred approach
• Comfortable interacting with patients and caregivers in a healthcare setting
• Healthcare, patient service or customer service experience is advantageous
