Locum Pharmacist | Weekend Only | $55/Hour 📍 West 💰 $55/hour 📅 Weekend Locum 🏥 Hospital-at-Home / Acute Care Setting

Job Responsibilities

• Review medications during patient transfers and assess medication supply requirements • Contact patients prior to transfer regarding required medication supplies • Answer patients' medication-related enquiries • Provide medication counselling through in-person and virtual consultations • Perform medication reconciliation • Review discharge prescriptions for appropriateness • Verify new medication orders prescribed by physicians • Identify inappropriate medications, dosages or medication-related issues where applicable • Support medication supply duties and follow up on medication preparation status • Work closely with the multidisciplinary healthcare team to ensure safe and appropriate pharmaceutical care • Perform other pharmacy-related duties according to operational requirements

Good Fit For

• Bachelor of Science in Pharmacy from a Singapore Pharmacy Council-accredited institution • Fully registered with the Singapore Pharmacy Council (SPC) • At least 2 years of relevant Pharmacist experience • Available to support weekend shifts
