Locum Pharmacist - Outpatient (Office Hours, central) #HAC

Salary: $30/hr Duration: 5 months Working Hours (for all positions):

• Monday to Friday: 8:30am – 5:30pm
• Alternate Saturdays: 8:30am – 1:00pm
• 5.5-day work week

Location: Novena Job Description: * Review transfer medications appropriately, contact patients pre-transfer on medication supply needed, and answer medicine related questions

• Medication counselling on transfer medications by a variety of means (in-person and virtual)
• Medication reconciliation
• Review discharge prescriptions appropriately
• Verify new orders by the physicians and identify inappropriate medication/doses if relevant
• Assist with perform medication supply duty
• Follow up on medication preparation status
• Undertake duties which may be assigned by the staff member depending on the requirements

Job Requirements:

• Min Degree in Pharmacy & VALID AHPC
• Min 2 yrs of relevant experience
