Locum Patient Liaison | $18/Hour #HHLT

💰 $18/hour 📅 3-Month Locum Contract 🕒 18–30 Hours/Week + Rotational Weekends / Public Holidays 📍 Work location may vary across institutions

Working Hours

• Weekdays: 8am–2pm
• Rotational Weekends & Public Holidays: 8am–11am
• Deployment and working hours may vary based on operational needs

💼 Responsibilities

• Engage and counsel patients and caregivers on the programme
• Explain programme requirements, care processes and hospital-at-home arrangements
• Verify patients’ contact and residential information
• Address patient and caregiver enquiries or concerns
• Facilitate suitable patients’ admission into the programme
• Escalate complex concerns to the clinical team where appropriate
• Coordinate with intake colleagues to ensure smooth patient transitions
• Maintain accurate records of counselling sessions and outcomes
• Support process improvements and other operational duties as required

📝 Requirements

• Min. diploma
• Able to commit to weekday shifts and rotational weekend / public holiday shifts
• Strong communication and interpersonal skills
• Empathetic and comfortable engaging patients and caregivers
• Able to manage multiple referrals and follow-ups in a fast-paced environment
• Comfortable working independently and within multidisciplinary healthcare teams
• Proficient with digital tools and able to learn new systems
• Healthcare, patient service or customer service experience advantageous
