Health Coach (1 Year Contract, GOVT Healthcare)

Location (Can choose): North / Northeast

Working Hours: 5 days, office hours

Occasional weekends: Required for community events

Contract: 1 year convertible to Perm

Job Responsibilities:

• Conduct health and care assessments to understand residents’ health status, needs and lifestyle risk factors.
• Engage residents in health goal-setting using the S.M.A.R.T. framework .
• Develop personalised health coaching plans based on individual needs and wellness goals.
• Guide residents in making sustainable lifestyle and behavioural changes.
• Support residents with chronic disease self-management and health risk reduction.
• Monitor, track and document residents’ progress and health outcomes.
• Work closely with the Community Health Team to coordinate holistic care plans and service delivery.
• Participate in community outreach programmes and health-related events, including occasional weekend events.

Requirements:

• Min A level or Diploma Nutrition / Dietetics/ Biomedical Science / Biomedical Engineering/ Nursing / Health Sciences or related field
