Your tasks

Job Scope

• * Provide care coordination and service linkage to clients with or who are at risk of mental health issues and/or dementia, with the appropriate resources and support including caregiver support.
• * Plan and conduct outreach to the community and stakeholders including faith-based, corporate and community organisations to promote mental wellness and mental health awareness.
• * Plan and conduct targeted mental health and dementia case finding/screening to promote early identification and facilitate timely support.
• * Plan and organize structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients.
• * Conduct regular visits to monitor the well-being of the client
• * Support in initial assessment of walk-ins and referral cases, follow up with intervention plans for clients as per the prescribed care plan.
• * Involve in the evaluation and documentation of the programme.
• * Any other duties as assigned by reporting officer or Head of Community Social Services

Requirements

Job Requirements

• Min. Diploma in Social Service, Psychology, Counselling or any relevant field
