6 Months Nurse Counsellor - HSN

We are hiring under Recruit Express headcount and you will be deployed to: Location: Central Working hour: 5 days (office hour) Salary: $29/hour Job Responsibilities Provide patient care and chronic disease counselling, including developing care plans, educating patients on lifestyle changes and medication adherence, and conducting diabetic foot screening and spirometry.

Coordinate with GPs and Primary Care Coordinators for patient referrals, care coordination, follow-up, and clinical documentation.

Support administrative and quality improvement projects, including clinical supervision and development of healthcare workflows and policies.

Requirements: Minimum Diploma in Nursing.

Registered with the Singapore Nursing Board (SNB).

Relevant nursing experience preferred.

Good communication and interpersonal skills.
