Care Coordinator

About the Role:

We are looking for a Care Coordinator to support patients throughout their healthcare journey across Raffles Medical Clinics and our wider healthcare network. You will coordinate appointments, referrals and follow-up care, while working closely with patients, doctors and healthcare providers to ensure continuity of care and a positive patient experience.

Key Responsibilities:

• Coordinate and monitor patients’ care journeys, including appointments, investigations, referrals and follow-up care.
• Liaise with patients, doctors, clinics, hospitals and other healthcare providers to facilitate timely care and follow-up.
• Track outstanding investigations, referrals and abnormal screening findings, and coordinate the necessary follow-up actions.
• Conduct patient recall and engagement activities for health screening, Healthier SG and other healthcare services.
• Provide patients with information and guidance on healthcare services, care pathways and recommended follow-up plans.
• Address patient enquiries and concerns and support service recovery where required.
• Maintain accurate patient records, tracking information and care coordination documentation.
• Support clinic operations, patient registration and administrative duties when required.

Requirements:

• Diploma, GCE A-Level or equivalent.
• Good communication and interpersonal skills with a patient-focused mindset.
• Proficient in English; knowledge of additional languages or dialects would be advantageous.
• Organised and able to manage multiple patient follow-ups effectively.
• Prior experience in healthcare, customer service or a service-related environment would be advantageous.
