Adult’s Social Worker, Hospital Team, Bournemouth

Pay rate £32.52 per hour Contract role, Hospital Team Pertemps are recruiting for an experienced Adult’s Social Worker in Bournemouth. This is to work in a Hospital Team in Poole. We are seeking an experienced Adult Social Worker to join the Hospital Social Work Team based at University Hospitals Dorset.

The post holder will undertake strengths-based assessments, support complex discharge planning, complete Care Act assessments, safeguarding enquiries, Mental Capacity Act work, and multi-disciplinary working to facilitate safe and timely hospital discharge.

Applicants must have experience of Adult Social Care and hospital discharge pathways.
