Care Coordinator

Responsibilities:

• Identify and engage patients for health screenings
• Educate patients on the importance of preventive health and screening.
• Coordinate care for patients with the chronic care team.
• Perform patient assessments, including vital signs and mental health evaluations.
• Document nursing care accurately in compliance with institutional policies.
• Make referrals for lifestyle modification programs as needed.
• Teach patients how to utilize health monitoring tools at home.
• Support health promotion activities in clinics and community settings.
• Assist in continuous quality improvement projects related to patient safety.

Requirements:

• Diploma holder
• Strong commitment to patient care and collaboration with healthcare teams.
• Willingness to participate in professional development and training.
• Capacity to handle multiple tasks in a dynamic environment.
