Overview and purpose of the position

The Case Management Officer is responsible for assessing, planning, coordinating, monitoring and tracking of cases to ensure continuity of care being delivered to patients. He/ She will need to collaborate with a multi-disciplinary team of doctors, psychologists, nurses, medical social workers, and other allied health workers in providing timely and appropriate treatment and mental health support for the patients.

The ideal candidate should have a passion for serving in the public sector to meet the mental healthcare needs of Singapore residents. He/she should be highly motivated to learn, grow personally and work independently. The Case Management Officer works under the supervision of the Case Management Officer Lead and the Programme Director.

Job Responsibilities

  • Support clinicians assigned to plan, execute and monitor care plans promptly including active collaboration with community partners, educational institutions, healthcare partners with the objectives to stabilize patients or improve clinical indicators
  • Provide prompt and progress updates to assigned attending physicians. Consult clinical supervisor or attending physicians promptly if needed.
  • Follow up with patients’/community partners queries promptly within same day for urgent cases; within 3 working days for normal cases
  • Provide prompt, clear and accurate documentation in Epic, patient database (for projects) and to attending physicians
  • Handling patient complaints and feedback
  • Conducting patient home visits
  • Maintenance of project databases and KPIs
  • Accepting GP/ Polyclinic and community partner referrals to outpatient clinic
  • Providing transitional care/ discharge of patients to community partners/ GPs
  • Case Discussion with community partners
  • Networking/Recruitment of with community/GP partners
  • Be involved in planning engagement meetings with new community partners
  • Work with a team of case management officers to enable the ASCAT/AFTERCARE case management team to meet clinical KPIs required

Qualification Required and Area of Discipline

Bachelor degree preferably in Nursing, Biomedical Sciences, Psychology or Healthcare

Required Competencies and Capabilities (Skills, Experiences and Professional Licences)

  • Minimum 4 years of experience in managing mental health conditions including 2 years in a hospital
  • Prior work experience in psychiatric facilities i.e. inpatient or outpatient psychiatric settings, and leading a team will be an advantage
  • Nursing background with advance/post graduate diploma in mental health will also be advantageous
  • Social work background will be helpful
  • Good computer literacy skills – MS Office (Word / Excel / PowerPoint)
  • Strong organizational, interpersonal and communication skills
  • Ability to communicate in local languages and dialects
  • Ability to multi-task and work independently and collaborate as a team
